Monthly Support Packages

These flexible and inclusive packages are available to individuals or families who are current patients of Be Well Clinic. The package spans a 30 day period, and can be started on any day of the month.

In order to provide you with quality support, we accept a limited number of subscribers to these support packages each month, with preference given to current subscribers. New monthly packages include a book and a journal to help you with your journey.

Automatic payment withdrawal is available, with a 5% discount per month. If you wish to cancel your monthly payment, it must be done with advance written notice (email is fine), at least 10 days prior to the automatic withdrawal date so the package can be made available for another patient or family.

Standard Patient Care: No monthly charge

  • Includes a subscription to the Be Well Clinic e-newsletter

  • Addition to the Be Well Families closed Facebook group

  • Allows you to list Amy as a primary care provider for any current patient

  • Gives access to communication outside of office appointments

    • Email, phone, Skype, Voxer: according to current fees schedule

    • Home visits are available, subject to availability

  • After-hours urgent phone consultations (dependent on provider availability)

 

Monthly Support: Basic                    $250/month

  • Includes a subscription to the Be Well Clinic e-newsletter

  • Addition to the Be Well Families closed Facebook group

  • Allows you to list Amy as a primary care provider for most current patients*

  • Weekly 30-minutes calls with Hollie, FNTP, CGC

    • 30 minutes of phone support per week

    • If an in-office appointment is required due to medical complexity, additional fees may apply

  • Allows participation in weekly GAPS support group call run by Amy (Thursdays 12:30-1:10pm MST)

  • Access to additional communication according to the current fee schedule

  • Additional phone consultations, as needed, depending upon practitioner availability. Subject to current fee schedule.

 

Monthly Support: Premium                    $500/month

  • Includes a subscription to the Be Well Clinic e-newsletter

  • Addition to the Be Well Families closed Facebook group

  • Allows you to list Amy as a primary care provider for most current patients*

  • Includes pre-paid, flexible communication with Hollie outside of office appointments

    • Weekly 30-60 minutes calls with Hollie, FNTP, CGC

    • Unlimited phone support

  • Allows participation in weekly GAPS support group call run by Amy (Thursdays 12:30-1:10pm MST)

  • Additional phone consultations, as needed, depending upon practitioner availability. Subject to current fee schedule.

*For Amy to be listed as a primary care provider for any child under the age of 12 they must have been seen in person by Amy within the last year.
